Bluetooth Issues

I have several issues concerning Bluetooth.
1. On main screen I see the popup dialog saying Bluetooth has stopped ... ( and some variations).
Repeatedly.
2. In the car, Bluetooth previously paired. Occasionally device will not connect to the car upon starting of the car. I try manual reconnect, connect telephone only, and it connects then disconnects.
3. I have been using a new device for the last few days, an electronic blood glucose meter. I use it several times each day. So today it failed to connect and sync data, I check Bluetooth settings on the phone and find it has absolutely no paired Bluetooth devices listed. Previously there were 6.
I try to fix the problem by restart the phone and also power down and power up again. This symptom remains.
4. I have had similar sync issues with my Samsung Galaxy S3 watch device. It normally connects to the phone to synchronise time etc and app data.

Summary
Clearly Bluetooth service has issues, I am wondering how this can be fixed.
