Sometimes apps are disappearing from main screens and you have to go to all apps menu to put again in the same place...

Got the same problem. Sometimes when i restart my phone some apps shortcuts on main screen are gone

Can you perform a factory reset and report back? Please don't install any 3rd party launcher. Keep your device at the stock state.

Some of them on SD, but most of them on phone
