Sorry to say that, but if the problem persists after a firmware downgrade + factory reset, it really looks like an hardware fault.
You should contact your selller for a replacement under warranty. Hardware faults can happen, also if they're annoying as hell.
This definitely ain't a hardware fault. Every system test app I've tried is able to activate the vibration, but there's something inside the firmware or the kernel that puts vibration to sleep for time to time, and apparently it will only wake up when there's new activity on the screen or the capacitive buttons
