I just want to emphasize a couple of points already made in this thread. I spent a good deal of time attempting to get various APN settings to work on two identical phones. I had MMS working, but not LTE on my own phone, but the same settings entered on my wife's phone did not give her either MMS or LTE.
Finally I read this thread more carefully and saw a couple of crucial points I had previously skipped over:
1. No matter what you do, you won't get LTE unless support at CC sets that option for your phone or phones on their end. Without that, you will be stuck with 3G or HSPA/HSPA+. Call support and specifically ask that they turn on LTE for the specific LTE-capable phone(s) you have on your account.
2. You really do have to start with a fresh, new APN entry. Changing values on an existing one probably won't work. Don't waste your time, just create a new one and change just the specific settings mentioned in this thread. Don't sweat the last one, MVNO Value. With a fresh APN, that value should have nothing in it, which is fine. If you screw up somehow, start from scratch with a new APN entry. Really.
Once I followed these two essential steps, everything quickly started working as it should.
